I agree to pay $350* (U.S. Dollars, USD ) NON-REFUNDABLE application fee and understand that due to the highly selective application process, there is no guarantee that the preferred electives or elective periods I submit on my application will be available. I understand my course(s) will only be guaranteed after UAB receives my course(s) payment in full.

I understand that if my application is accepted, I will have to pay $5,200* (U.S. Dollars, "USD") to cover my housing, transportation from and to the Birmingham airport, course fees, malpractice insurance and other administrative cost related to my elective at UAB.

I understand that UAB International Medical Education reserves the right to modify, postpone, or cancel a course at its sole discretion at any time due to administrative, operational, scheduling, or unforeseen circumstances. Should UAB International Medical Education cancel a course for any reason, participants will receive a full refund of the course fee paid, which includes tuition, UAB IVMS housing for the duration of the course, and transportation between Birmingham-Shuttlesworth International Airport (BHM) and UAB IVMS Housing. UAB shall not be responsible for any costs or expenses not included in the course fee.
