Course Description This 7-day long exercise develops empathy within learners for those patients with diabetes through the participation in daily activities that individuals with the disease state are often encouraged to conduct by the healthcare team. Goal: To improve empathy for patients with diabetes in hopes of positively altering the learner's professional interactions with this population in the future. Objectives: By fulfillingthe week-long diabetes experience and composition of the reflective piece, the learner is expected to: 1. demonstrate ability to perform specific diabetes-related self-management tasks (carbohydrate counting, SMBG via glucometer use, insulin dose calculation, insulin injection technique, and implementation of daily exercise) 2. identify challenging components of the standard diabetic regimen 3. consider hindrances associated with long-term continuation of self-management efforts 4. develop empathy for those living with diabetes. Course format: to be conducted over seven (7) consecutive days (the student may select which day of the week to begin the Diabetes Experience). - Feet exams: check your feet daily and exercise for 30 minutes most days of the week. - Physical activity: Initiate an exercise routine consistant with the American Diabetes Association recommendations for 30 minutes on most days of the week. - Diet: Count the number of carbohydrates eaten over the course of the week. Document the CHO intake of each meal in a log. - Blood glucose testing: Self monitor your blood glucose(SMBG) BID before 2 meals each day and five times during the week 2 hours after a meal, and at least once at 3 am. - Insulin administratiaon: Add 100/mg.dl to each SMBG value received (fasting and post-prandial). Pretend that this is your tested blood glucose value. Assume your insulin sensitivity index is 1.10 and that your insulin toCHO ratio is 1.5. Calculate the number of units of rapid-acting insulin needed to compensate for the CHO intake you are about to consume (if fasting) and the amount of insulin needed to adjust youir BG to 120 mg/dl. Using the placebo insulin pen administer the calculated number of units into the stress ball after each SMBG test, whether it is before eating meals, post prandial, or in the middle of the night. Document all of these activites in a weekly log (template provided). - Insulin regimen: At the end of the week reflect on your insulin dosing requirements and develop an improved insulin dosing strategy based on your required insulin doses for the week. Document this improved insulin dosing strateghy at the end of your weekly log. - Patient interview: Interview 3 individulas who have diabetes and are unrelated to each other regarding their ability to maintain healthcare recommendations for the disease state. This should not be a standard medical interview; rather the intent should be to learn how they live with, how they view, and how they cope with disease. - Documentation: - Type a 2 page (2 complete pages in length) single-spaced reflective piece on the experience due within 4 days (not excluding weekend days) from the completion of the experience. The paper should be typed using 12 point font or smaller, single spaced, using margins, with a self-determined and unique title; course name and number, student name and month/year of the rotation. - Paste the log of your activities after your 2 page reflective piece. The log should include your CHO counting, SMBG values, calculatled insulin doses, exercise routine, feet checking, and patient interviews. - Develop an improved insulin dosing strategy based on required insulin dosing regimen for the week.
